Utica Park Clinic (UPC) social workers have provided assistance for many patients and they would be glad to help you, too.  They can meet with you privately to help assess your needs and determine what psychological, social or financial challenges you may be facing. Regardless of whether those challenges are due to a medical condition or affecting a medical condition, they can provide the appropriate level of assistance in areas such as:

  • Addiction
  • Basic needs such as food, utilities, and transportation
  • End-of-life planning
  • Depression & anxiety
  • Divorce
  • Grief counseling
  • Housing, including nursing home or assisted living placement
  • Illness (mental or physical)
  • Long-term care planning
  • Stress
  • Support for victims of child or elder abuse
